    Council’s Aged and Disability Services play an important

    role to ensure members of our communities can remain

    living at home on an independent basis.

    Council is seeking to appoint suitability qualifi ed and

    experienced candidate to undertake the role of Packaged

    Care Case Manager. The position is responsible for

    providing support and advice to recipients of Care

    Packages (CACP, EACH & EACHP) within the Shire.

    The ideal candidate will have a friendly nature, good

    communication skills and a proven ability to work on an

    individual and team basis.

    Experience in working with people who are aged, frail,

    have a disability or dementia and their families is essential

    to succeed in this position. A relevant qualifi cation within

    the Health or Community Services fi eld is also required.
